Introduction:

This medication is commonly used for the treatment of high blood pressure, it also helps to prevent blood strokes, heart attacks, and kidney problems. it is taken by oral route of administration(mouth). Telmisartan belongs to the class of drug called angiotensin receptor blocker. The main mechanism of the drug it works by relaxing blood vessels so blood can flow more easily. Telmisartan medication is taken with combination drugs to treat high blood pressure effectively.

How to take:

This medication is highly recommended for high blood pressure. Take this medicine by mouth (oral administration )with or without food as directed by your doctor, once daily. The dose is based on your medical condition and responses to the particular treatment. Use this medication regularly to get the most benefit from it. to make you remember, take it at the same time each day. 

Storage condition:

Store telmisartan at room temperature from 56-89 F. keep it away from high and low temperatures. 

Duration between two doses:

Administration – orally(mouth)

Dosage first must be individualized. The usual starting dose of telmisartan tablets is 40mg orally once a day. Blood pressure response is dose related over the range of 20mg to 80mg. most of the effect is apparent within 2 weeks and maximal reduction is generally attained after 4 weeks. No initial dosage adjustments is required elderly patients or patients with renal impairment, including those on hemodialysis.

Oral telmisartan follows nonlinear pharmacokinetics over the dose range of 20mg to 160mg. both present greater proportional increases at higher doses. With once-daily dosing, telmisartan has through plasma concentration. Telmisartan has a volume of distribution of approximately 500 liters.  Minimally metabolized by conjugation to form a pharmacologically inactive acyl-glucuronide.	Telmisartan is an orally active nonpeptide angiotensin 2 antagonist that acts on the AT receptor subtype. It has the highest affinity for the AT1 receptor among commercially available ARBs and has minimal affinity for the AT2 receptor.

Side effects:

• Dizziness

• Extreme fatigue 

• Back pain 

• Blurred vision

• Diarrhea 

• Headache

• Swelling 
  

Telmisartan may reduce the amount of urine you produce or increase your risk for kidney injury. There are no serious side effects of this tablets. If occurred immediately consult your physician.

When to see doctor:

1. Telmisartan is mainly used for the low blood pressure. If your pregnant or planning to become pregnant tell your physician about it. 

2. if you become pregnant while you are taking this medication, stop taking it and contact immediately to your doctor. 

3. Telmisartan may cause death or serious injury to the fetus when taken in the last 6 month of pregnancy.  

Dosage administration:

Tablets – as per USP available 20mg, 40mg, or 80mg. 

For hypertension – 40mg/day PO initially. Patients with low volume depletion should receive the lower dosage initially, under close supervision  

For cardiovascular risk modulation – 80mg/day PO initially, indicated for cardiovascular risk modulation in patients unable to take ACE inhibitors 

 Important note:

• Check regularly your blood pressure while taking this medication. Learn how to monitor your own blood pressure at home, and share the results with your doctor.

• Tell your physician if you have severe diarrhea or vomiting. To prevent dehydration, drink plenty of water unless your doctor tells you not to.

• This medicine may increase your potassium levels. Before using potassium supplements or salt substitutes that contain potassium, consult your doctor or pharmacist.
